    I am not sure just where this is but was thinking it is from Beaver Dam,  Wisconsin
    This name plate is made of plastic and measures approx. 5 5/8 inches by 1 1/2 inches.  The adhesive tape that was on the reverse side had deteriorated so it was removed
    It is in good condition .
